We accept most PPO plans.

Before your first visit, we will review your insurance coverage and inform you of any applicable fees.

To assist us in assessing your insurance coverage, please provide us with the following information so we can expedite your reimbursement:

• Your dental insurance provider
• The primary subscriber on your dental insurance
• The primary subscriber’s date of birth
• The primary subscriber’s identification number

We retrieve your insurance information before your first visit, check the status, and inform you of necessary copay, if any, before your visit.

Our friendly, well-informed staff will be glad to prepare and submit dental insurance claims and assist in making insurance collections for all patients with insurance benefits.

Payment is due at the time of your visit. Any remaining balance after your insurance company has paid is the patient’s responsibility and is due at the time service is rendered.
